Checklist to ensure your booking is complete:

All passengers must complete the following forms. 

1.) The Dive Expedition Application may be filled out electronically and emailed or printed out and sent by post or fax. If it is within 60 days of your charter, please use email or fax.

To fill out the form electronically, you will need Adobe Acrobat Reader 7 which is available for free download from http://www.adobe.com/products/acrobat/readermain.html

2.) The second part is the Liability Release and Diver or Non-Diver Liability Agreement. This part must be signed and sent by post or fax. If it is within 60 days of your charter, please use fax.

Payment and cancellation policy : A 50% deposit is required to make a booking with the balance payable 60 days prior to departure. Full payment is required for bookings less than 60 days prior to departures

Credit Card Authoritation Form If you are making a payment using a credit card, please print and fill out the appropriate Credit Card Authorization Form and fax it to us in  Fiji: (++ 679)-65 13 73 7

We recommend you take out travel insurance and Emergency Medical Evacuation Assistance, prior to your departure.
